The Outreach Case Manager provides direct, trauma-informed case management to youth ages 18–24 accessing the Drop-In Center. This position supports clients in navigating housing, employment, benefits, and life skills resources. The Outreach Case Manager builds trusting relationships with youth, supports crisis interventions, collaborates with program staff, and ensures all services are documented in alignment with agency and funder standards. This role is integral to connecting unhoused young adults to stable, supportive solutions.

Qualifications
  • Bachelor’s degree required; Master’s preferred (Social Work, Counseling, etc.).
  • 3–5 years of case management experience with at least one year in a leadership role.
  • Strong organizational and interpersonal skills.
  • Familiarity with trauma-informed care and positive youth development.
  • Prior experience with HMIS preferred.
Essential Functions
  • Champion YMCA core values of honesty, respect, responsibility, and caring.
  • Supervise case management interns.
  • Provide consultation, support for difficult cases, and help with ethical problem solving.
  • Ensure consistency of documentation across systems (HMIS, internal forms).
  • Provide onboarding and training for new case managers.
  • Coordinate with Outreach, YSTEP, and Housing teams to ensure service alignment.
  • Maintain compliance with licensing, confidentiality, and grant standards.
  • Track program outcomes and support reporting needs.
  • Provide case management to Drop-In Center clients experiencing homelessness
