JOB SUMMARY:

Implement, maintain and expand the YMCA's Chronic Disease and Healthy Living programs to include Diabetes Prevention Program (YMCA DPP), Cancer Wellness, Blood Pressure Self-Monitoring, Pedaling for Parkinson's, Weight Loss and Enhance Fitness in all four counties of the YMCA of the Suncoast service area. Supervise daily operations of the above mentioned programs and provide support and guidance to program instructors and coordinators and ensure that the program achieves quality performance outcomes.e deserve the best, the YMCA offers exceptional wage and benefits package.

High School graduate or equivalent required. Four-year college degree in health, medical or related field preferred. At least two years of previous health related program development or related experience required. Advanced knowledge of computer software applications (word processing, spreadsheets, and graphics) experience in a Windows environment required. Knowledge of basic health, nutrition, and fitness is encouraged and understanding of behavior change required. Familiarity and proficiency with Listen First or motivational interviewing highly encouraged. Must obtain and maintain YMCA of the USA community health program certifications and others as assigned. Broad overview of the YMCA as it operates as a local, national, and international movement preferred.

A commitment to and supports the Y mission. Must have outstanding communication skills; exceptional organizational skills; a passion for data and details; flexibility, and the ability to build strong relationships, inspire confidence, and work effectively with a wide variety of stakeholders (physicians, public health officials, employers, payers, health seekers, and senior Y leaders and instructors). Public health background a plus.

May be required to lift and carry up to 30 lbs in the form of boxes of program supplies. May be required to sit or stand for extended periods of time. Receives and follows general instructions. Must be capable of working under pressure as well as communicating both verbally and written when dealing with various members of the community. Must possess strong decision making and analytical abilities. Must possess auditory, visual and verbal abilities to communicate via phone and in person.

Essential Functions

High Quality Programs, Services and Facilities

  • Monitor the quality of support that instructors provide to program participants.
  • Monitor the fidelity and quality results of medical based wellness programs.
  • Recruit, screen and register eligible participants into community health programs.
  • Organize master schedule of community health programs.
  • Respond to inquiries from the general public and members of the public health, physician and payer communities about community health programs.
  • Become certified to deliver and/or be a trainer for Community Health Programs.
  • Implement and supervise processes at all program sites to ensure patient privacy in accordance with HIPAA.
  • Maintain CDC and Medicare Recognition for YMCA's DPP.
  • Assist with retention and commitment of program participants, track membership conversion across all programs.
  • Regularly analyze data from community health programs to ensure they meet quality performance standards.

Staff and Volunteer Management

  • Hire, manage and supervise program instructors, coordinators and other staff as assigned.
  • Assist in the recruitment and organization of a group of volunteers for the YMCA Healthy Living committee.
  • Provide support and leadership to wellness directors in planning annual cancer survivor wellness program events.

Finance

  • Monitor third party payer reimbursement for community health programs.
  • Develop and manage yearly budgets for community health programs.
  • Manage grant budgets and deliverables.

Community Development

  • Establish relationships with public health, physician, and payer communities and other referral networks to enhance awareness of and referrals to the YMCA Community Health Programs.
  • Serve as a liaison, ambassador, and advocate for the YMCA and other community health programs within public health, physician and payer communities.
  • Engage partners in the health communities throughout all 4 counties of the service area.
  • Work closely through the state alliance and with other Florida YMCA's to reach out to large employers and payers.
  • Engage marketing staff to ensure adequate publicity and marketing of YMCA Community Health Programs.
  • Engage senior Y leaders to be ambassadors and advocates for Community Health Programs in the public health, physician and payer communities.
  • Assist senior Y leaders in leveraging relationships with public health, physician and payer communities for the benefit of the YMCA.
  • Work closely with the other YMCA's in the Tampa Bay area to maintain seamless service to third party payers and shared employer groups.

Financial Development

  • Manage deliverables for all grant funded projects.
  • Seek out opportunities in funding for program sustainability.
