The MetroWest YMCA is seeking a full time Director of Chronic Disease Prevention Programs. Exempt annual salary $50,000 - $60,000 based upon experience and credentials! Competitive benefits! Equal Opportunity Employer.

The full time Director of Chronic Disease Prevention Programs based in Framingham, MA is responsible for managing all referrals and classes for all chronic disease prevention programs including but not limited to: Diabetes Prevention, LiveStrong, Parkinson’s Wellness Program, Matter of Balance, Walk With Ease, Healthy Weight and Your Child, Blood Pressure Self Monitoring Program and Enhance Fitness.

Additionally, this position will act as a liaison for seniors moving through evidence-based programs to traditional wellness programs at the Y, supporting them and their families along the way. This position would plan special events and programs to increase member connections and decrease social isolation in our older adults.

This position requires direct delivery of interventions and participation in quality improvement cycles to help implement best practices.  Responsibilities also include direct delivery of other adult and youth programs and service as needed to support the wellness department. Some weekend hours are required. 

Qualifications

 1. Bachelor's degree in public health, health promotion, exercise science, disease prevention or a related field preferred.

2. Current national certification in group exercise, personal training and CPR/First Aid.

3. Two or more years of related experience to include database management, data collection, referral coordination and/or program development.   4. Computer skills to include: Microsoft Access, Microsoft Excel and Google Drive.   5. Competency in relationship building skills, communication skills, and project management.   6. Multilingual or bilingual a plus.
Essential Functions
1. Manage all referrals for chronic disease prevention programs in google referral system or another system. · Schedule and coordinate all chronic disease prevention programs · Maintain accurate records, files and data collection procedures · Assist in analyzing program data and implementing quality improvement cycles · Aim to grow all of these programs in participation and referrals · Directly supervise staff leading prevention and wellness programs · Assist in grant proposals for programs   2. Develop and maintain collaborative relationships with clinical and community partners to enhance awareness, increase referrals to the program and secure space for programs.   3. Provide direct delivery of chronic disease prevention and management interventions as required. 8-10 hours of direct delivery. Onsite as well as offsite.   4. Maintain all certifications and attend trainings/Wellness meetings as necessary for the position.   5. Delivers a minimum 5-6 hours per week of additional direct delivery of adult and youth wellness programs, personal training, consultations, wellness floor hours, small group training, Weigh to Change, Healthy Weight and Your Child and group exercise classes.   6. Track Membership conversions from program participants to members, work with the Director of Family Engagement to promote membership to participants   7. Research new opportunities for grant funding and new programs for the Y. Stay up to date on latest research on chronic disease prevention and organizations doing similar work.   8. Models relationship-building skills (including Listen First) in all interactions. Responds to all participant and community inquiries and complaints in timely manner. 
Required Certifications
