Job Description

The Case Worker is responsible for assessing needs and strengths of families at risk of out of home placement, adhering to the principles of the Wraparound Model, the underlying intervention philosophy of Interagency Family Preservation Services (IFPS) program. The Case Worker will also be responsible for monitoring services provided by other agencies and finding informal supports for families from recreational activities, mentoring and connecting with faith community if requested by the family.

THIS POSITION IS AN IN-PERSON POSITION AND REQUIRES IN-HOME VISITS. PERSON HIRED MUST POSSESS AN ACTIVE SOCIAL WORKER LICENSE IN THE STATE OF MARYLAND (LBSW, LMSW, LCSW-C)

Qualifications
  • REQUIRED: License Bachelor Social Worker (LBSW), License Masters Social Worker (LMSW), or License Certified Social Worker (LCSW-C) from an accredited college or university.
  • Must maintain licensure in good standing in Maryland with the respective licensing board.
  • Bilingual English/Spanish preferred.
  • Experience working with youth and parents in a multicultural community, with diverse populations, while collaborating with multiple agencies preferred.
  • The Case Worker must demonstrate excellent written and verbal communication skills and personal values consistent with the YMCA mission.
Essential Functions
  • Provide time-limited, home-based case management, psychoeducation, advocacy, parenting skills education, and intervention services to address factors that place children at risk for out of home placement.
  • Provide intensive services of at least 5 hours a week of face-to-face care coordination for a maximum of 6 weeks with a transition to a step-down level of an average of 2-3 hours a week of face-to-face care coordination for a maximum of 8 weeks.
